       Question: Card activation  
      
Answer  : The main cardmember can contact Cardmember Service Center to activate the card at the automatic phone number
02-627-6266 via automatic phone system by pressing menu 2.
 
     
      Question: Account statement copy  
     
Answer  : Call Cardmember Service Center at 1712. After choosing the language, press 3 to select whether to deliver the statement
by fax or by mail.
 
     
       Question: Supplementary card application  
       Answer  : The applicant must be 15 years old or more. Use the supplementary cardholder’s personal identity card copy for
application.
 
     
       Question: Request application form status  
      Answer  : Call Cardmember Service Center at 1712. After choosing the language, press 4.  
     
      Question:  Lost card  
    Answer  :   Call Cardmember Service Center at 1712. After choosing the language, press 1.  
     
       Question: TESCO Lotus Visa Card Service Center phone number and office hours  
    Answer  : 1712 Office Hours: Monday - Friday 8.30 hrs. -22.00 hrs. Saturday – Sunday 10.00 hrs. -19.00 hrs.  
     
      Question: Card application process  
     Answer  : Immediate approval at TESCO Lotus or send the documents for further approval at the address as stated on the application form.  
     
       Change TESCO Visa Accumulated Points to Club Card Cash Voucher  
     
      
Question: Cardmember transfers TESCO Visa accumulated points for changing into Club Card cash voucher  
    Answer  : The customers who applied and got approval from June 16, 2011 onwards and the customers receiving letter notifying
the change into Club Card cash voucher.
 
     
      
Question: Computation of TESCO Visa accumulated points for changing into Club Card cash voucher every 3 months  
      Answer  : For your every 250 accumulated points, TESCO Visa will transfer them to Club Card equivalent 2,500 Club Card points for changing into Club Card cash voucher valuing THB 25.  
     
      
Question: Checking the change of accumulated points to Club Card cash voucher  
 
Answer  : You can check TESCO Visa accumulated points transferred to Club Card in the credit card account statement displayed in “Points Transfer Out”, which will add to Club Card accumulated points shown in TESCO Lotus Visa Card on Club Card letter.  
     
     
Question: When member wants to change Club Card number for transferring the accumulated points to Club Card cash voucher  
 
Answer  : Fill in the form for changing Club Card number and attach a copy of the new Club Card and TESCO Lotus Visa Card. The new Club Card must belong to you. Then call 1712 and press 1.  
     
       Club Card Information  
     
    
Question: What is Club Card?  
      
Answer : Points accumulation card for spending at TESCO Lotus. When the customers accumulate the points up to the determined date within 3 months, he/she will receive cash voucher in proportion with his/her accumulated points by mail. For more information, please study at www.tescolotus.com  
     
 
Question: Where to apply as a Club Card member?  
      
Answer : Apply at every branch of TESCO Lotus (Hypermarket, Value Shop, Lotus Market and Lotus Express) throughout Thailand.  
     
    
Question: What to do when the customer lose the card?  
      
Answer : The customer can check at the branch whether any staff keeps the card. If not, the customer can contact for new card issuance immediately at Customer Service Point by simply fill in 3 blanks in the application form e.g. put a tick mark on “old card replacement”, number of the lost Club Card and the customer’s signature at the bottom of the application form (If you cannot remember, please contact Club Card Service Center at 1712, then press 3 for searching the old Club Card number).
